A family has shared their heartbreak after a ‘kind and loving daughter’ died after staying at a Greater Manchester mental health unit.
The 15-minute observation plan had been instigated after Cerys had experienced multiple incidents involving ligatures on the ward, her family has said. But sheets of handwritten notes about the 15-minute observations that were meant to take place from 1pm to 3pm on May 13 are ‘missing’, a pre-inquest review into Cerys’ death was told.
In a moving statement shared with the M.E.N following her death, the Lupton-Jones family spoke to their beloved daughter, saying: “Lovely Cerys, you have touched so many people in your short life, far more than you probably ever realised, and we will cherish these memories in our hearts forever.” After GMMH was last inspected in July 2022, mental health chiefs were hit with two warnings – demanding improvements in safety and low staffing levels. The year after, the health watchdog slammed the trust as inspectors again found ‘a continuation of concerns regarding the safety of people which need to be addressed as a matter of urgency’.
“We are committed to keep progressing our improvement plan. The inspections took place earlier this year and I can see even during my short time here that a tremendous amount of work has taken place and will continue, to make the required improvements." “Cerys loved sports, enjoying gym club, tennis lessons and from the age of six to 16 played football with local team Hurst Colts. She also began playing cricket in her teens and excelled at that too.
