Bleach's prototype killed off a major character immediately.
Before a manga series like Bleach is picked up for serialization, it's common for the creator to make a "pilot" chapter, which establishes what the main character, setting, and tone of the series will be. For many series, things don't change too much, and that pilot chapter becomes the first chapter. Other series, however, end up with a brand-new first chapter because things have radically changed, which is exactly what happened to Bleach.
SCREENRANT VIDEO OF THE DAY SCROLL TO CONTINUE WITH CONTENT The pilot chapter of Bleach features a number of differences from the series proper. Some are relatively inconsequential; for example, Rukia was originally a member of Squad 3, rather than Squad 13, and Ichigo had black hair for the first few pages. Others, like Rukia shrinking down to the size of a doll when her power is depleted, are hilarious and absurd.
The Original Fate of Orihime Inoue In the pilot chapter, Ichigo's family runs a mortuary rather than a medical clinic. Ichigo receives a call asking for services for a 16-year-old girl: his classmate, Orihime. As a Soul Reaper, Ichigo goes to send Orihime's spirit to the Soul Society by Rukia. Orihime apparently died falling down the steps of a shrine, and when they arrive at the location, her spirit is shaking her own body, trying to get back up.
Fans will immediately recognize some elements of this which made it into the story proper; instead of her father, it's her older brother who becomes a Hollow in the main series, and he voices many of the same complaints about her forgetting him. However, in the main series, Orihime's Hollow brother wasn't able to actually kill her, as it was still possible to return her to her body.
From the perspective of a fan at the end of Bleach, it's difficult to imagine just how different the series would've been had it stayed closer to the pilot chapter. Many of the elements that made the series, so popular were already present, so it may have succeeded regardless.
