Frank de Jong took himself to the emergency department of a major Tasmanian hospital for some tests; not long after that he was sent home where he died just hours later. Frank's grieving family want accountability from the hospital — and they are not alone, with many chasing answers.

Just months after a senior staff member at a major Tasmanian hospital was found to have unlawfully certified dozens of deaths, another family is making allegations against the hospital.

"His GP said they did some tests and something had shown up, an indicator that he would need to go and get more tests done," Mr de Jong said."We spoke to him on the phone, he was in the ED in bed, getting the tests done." He said staff at the LGH were "being evasive" and said it appeared as if they were "hiding things … like they're trying to cover themselves as a priority, over letting the family know information".

Staff and families had concerns about death certificates at a hospital. Now, 29 have been referred to the coroner "Patients should be able to nominate others who may be able to access their medical records if and when they're unwell or even when they are deceased."
