A coroner has delivered her findings into the death of a baby girl thrown into the Tweed River by her father in 2018.The coroner said it was of considerable concern that the child had fallen through the cracks.

New South Wales Deputy State Coroner Harriet Grahame has handed down her findings from a 2023 inquest into the death of the nine-month old, known as Baby Q. The family, including the baby's older brother and mother, was homeless at the time and had moved several times between Queensland and northern New South Wales.

The inquest heard the family had had contact with welfare agencies in the weeks and months prior to the baby's death, but it was concluded that there were no immediate safety concerns for the children.The coroner said while she knew very little of the baby's personality, photos showed her as "chubby-cheeked and smiling".In delivering her findings, the coroner said the family's transience had impacted their ability to receive support.
