Liam Slade, from Newport, is due before magistrates accused of causing death by dangerous driving.
The crash, in July, caused significant delays on the Prince of Wales bridge and into WalesThe two-vehicle crash happened between junctions 22 and 21 near Bristol in the early hours of Monday 17 July.Liam Slade, 33 and of Aberthaw Circle in Newport, has been charged with two counts each of causing death by dangerous driving and causing death by driving while disqualified.Mr Slade is due to appear at Bristol Magistrates' Court.
Three other men arrested as part of Avon and Somerset Police inquiries have been released without charge.
