The Herald's View: Mistakes in NSW regional hospitals have had a devastating and long-lasting impact on the families of those affected
The purpose is not to heap blame on the medical teams involved. They are no doubt doing their best in difficult circumstances.
The cases include: an 82-year-old man who died after a fall in Macquarie Base Hospital and staff did not carry out a mandatory falls risk assessment; a 66-year-old woman who died in Orange Base Hospital after a blood vessel was punctured during an unnecessary biopsy performed while she was fully conscious; the death of a woman with a torn oesophagus following elective surgery; and the death of a man with fatally high potassium levels after hospital staff did not act on abnormal pathology...
There are questions, too, about whether NSW Health and local health districts are doing enough to investigate cases that fall below the threshold for launching a full coronial inquiry.A report in May on rural, regional and remote health by NSW parliament’ssaid there was a “culture of fear” which did not encourage or value feedback and complaints. Some workers say they were even punished for making complaints.
The report by the parliamentary committee recommended a complete overhaul of the complaints mechanism.
