I work at a community service in Gippsland, and twice in the past three weeks have come across people discharged from hospital without sub-acute, post-discharge home care. Both were discharged to relative homelessness.

My daughter is helping but lives 90 minutes away, so I may have to shop around for alternate care. It is poor that I have not received even a phone call from Mecwacare.
