Almost 400 officers were at the scene of the Uvalde school shooting in Texas before police intervened, according to an official report.

The 77-page document, by the Texas House investigative committee, found the incident involved “systemic failures” and “egregiously poor decision making”.

“At Robb Elementary, law enforcement responders failed to adhere to their active shooter training, and they failed to prioritise saving the lives of innocent victims over their own safety,” it said. “Despite the immediate presence of local law enforcement leaders, there was an unacceptably long period of time before officers breached the classroom, neutralised the attacker, and began rescue efforts.

“We do not know at this time whether responders could have saved more lives by shortening that delay.” The report described the shooting which resulted in the killing of 19 children and two teachers as a “horrific massacre”.
