See the safety feature being added to float planes to prevent repeat of Mutiny Bay crash

A final report issued by the NTSB confirmed investigators early suspicion that an equipment separation led to the 2022 Mutiny Bay Crash that killed 10 people.

investigators' early suspicionrecommends the Federal Aviation Administration require operators of the DHC-3 airplane to install a backup connector to the piece known as an 'horizontal stabilizer actuator', which the report says came apart in flight and sent the float plane into an uncontrolled dive.

“We took a look at what we could do to help prevent the clamp nut from unscrewing should something happen to a lock ring." said Eric Ellison, the director of maintenance for Kenmore Air. “Even without the lock ring, the safety wire would prevent any rotation. We were just able to add a redundant safety feature."

Gudgel said the float plane industry is a close-community and the crash has impacted all DHC-3 operators in Washington.
