An internal WA Police investigation revealed eight officers failed to properly assess the risk posed by Mark Bombara, who fatally shot his wife's best friend and her daughter before taking his own life. Ariel Bombara, Mark's daughter, spoke out about her family's repeated warnings to police about her father's escalating abuse and access to firearms, claiming they were ignored. The investigation led to the implementation of new firearm laws and a dedicated family violence coordinator within WA Police.

An internal investigation into the actions of WA Police officers in the weeks before Mark Bombara killed two women in the Perth suburb of Floreat has found eight officers did not perform their duty. Mark Bombara shot dead his wife's best friend, Jennifer Petelczyc and her teenage daughter Gretl, in their home last May before turning the gun on himself.

"Red flags that seemed so obvious to me that it baffles me that actual police officers, whose job it is to protect people, did not take this risk seriously."This was the perfect storm of apathy and compassion fatigue from officers combined with shoddy systems, outdated policies and insufficient training and support that resulted in a lethal outcome."They should not have had to die in order for the police to start taking domestic violence seriously but here we are.

"I hope that the recommendations from this report are a good first step in fixing a broken system that failed multiple families and eight police officers and continues to fail women and children today," she said.Ms Bombara said she appreciated the work that had gone into the investigation and that she had now been listened to.Commissioner Blanch said officers failed to correctly assess the risk against the Bombara women when they raised their concerns.

The details of the disciplinary action taken against the eight officers has not been revealed, but Mr Blanch confirmed none had lost their jobs. She told of how she and her mother warned police three times about the danger her father posed, raising concerns about his collection of 13 guns. "I fought for eight weeks against every person that tried to convince us we were overreacting," she said.
