An inquest into death of a man from septic shock at Bunbury Hospital highlights miscommunication and significant backlogs the state's struggling regional health system.
Colin Nicholson was a husband, dad and grandfather, and volunteer.A coronial inquest has been held in Bunbury into the death of Collie man, Colin Stanley Francis Nicholson .
"Colin had a wealth of knowledge and was an esteemed trainer and assessor, as well as several other roles within the SES," Chantal Franklin, Mr Nicholson's daughter said. "It's incredible to think the same day Mr Nicholson's referral was opened, was the same day he died," Coroner Linton said. An internal review suggested the first two samples were interpreted incorrectly and not properly referred to a haematologist.
By that afternoon, Mr Nicholson presented to the Collie Hospital with a headache, a high fever and shivering. "A day makes all the difference in any form of sepsis, but with neutropenic sepsis when an immune system is disabled, getting antibiotics is essential," Dr Mountain said.Since Mr Nicholson’s death, WA Country Health Service executive director of clinical evidence Dr Helen Van Gessel said the service had placed more emphasis on training the recognition of febrile neutropenic sepsis.It was also unclear how Mr Nicholson came to be discharged from hospital at 6pm that day.
