A teenager has taken their own life inside WA's controversial youth detention centre Banksia Hill, the ABC understands, becoming the second child to die in youth detention in the state in less than a year.

Last October, 16-year-old Cleveland Dodd became the first person to die in youth detention in WA's recorded history. An ongoing inquest into Cleveland's death has heard mountains of evidence about how the crisis in youth justice reached the point where Cleveland slipped through the cracks.
