An inquest into an infant who died at a Sydney family daycare finds he suffered from an undiagnosed heart condition but that 'unsafe' sleeping conditions contributed to his death.

The Coroner was also scathing in his remarks about Kidstart, the company responsible for the centre Jack died at.

"Ms Shamsin failed to take ownership of responsibilities which she bore as director of Kidstart … [she] repeatedly sought to deflect these responsibilities onto others."Kidstart supervisor, Faten Trad, who was responsible for the company's day-to-day operations was found to have kept visits to the Randwick centre brief, at one point even leaving her car on in the driveway.

Joe and Margot Loh have called for the Department of Education to overhaul its assessment and auditing processes for childcare. In July 2015, inspectors visited a number of Kidstart premises and found that the service was not meeting standards in all key areas bar one.
