Four children aged between six months and 15 years old were violently killed in Victoria between 2016 and 2017. A coroner has called for the creation of a Child Protection taskforce in the wake of a damning report into the deaths.

"The initial three reports made prior to child one's birth concerning her siblings were all closed at the intake phase," Mr Cain said. "If that had occurred, Child Protection could have sought his consent to share his adverse criminal record with , and might have been engaged with the Stronger Families program and other relevant services," Mr Cain said.

The inquest recommended the Victorian government create a workforce to investigate the challenges faced by social workers and Child Protection staff. The second case the coronial inquest heard about was a six-month-old Aboriginal baby who died after being assaulted by their mother’s partner. The final case that the inquest heard about was that of a 13-month-old child born to Somali immigrant parents. The inquest heard the mother struggled with severe mental health issues, including postpartum psychosis, which led to multiple hospitalisations and Child Protection reports.
