A NSW Firearms registry official looked at a police report containing 18 entries of John Edwards’ history of assault, stalking, and AVOs, before giving the green light to allow him a permit that allowed him access to guns, an inquest had heard.

On 5 July 2018, Edwards stalked his children to the West Pennant Hills home where they were living with their mother, Olga Edwards, and shot them to death.He took his own life a few hours later.An inquest into the deaths of Jack, 15, and Jennifer, 13, is examining, among other things, how Edwards was granted a gun license and access to guns, given his 30-year history of domestic violence offences.Olga Edwards had reported her former partner's abuse of the children to police.

An audit of the clerk’s use of the database showed she had clicked into links relating to the 2000-2003 AVO one of Edwards’ former partners had taken out against him. Ms Richardson also referred to fresh allegations Olga had made against Edwards, with whom she was in a bitter court custody battle, that he had assaulted the two children he would go on to kill.
