A coroner has ruled what contributed to a baby’s death at a Sydney daycare centre after he was put down for a nap.
Deputy State Coroner Derek Lee on Wednesday delivered his findings into the death of Jack Loh, who was found unresponsive after being put down for a nap in March 2019.
Following an inquest in June and August earlier this year, Mr Lee on Wednesday said the bassinet in which Jack was placed was not suitable for a baby of his age.As well, the bassinet also contained a pillow and a sheet and Jack was still wearing a bib and his clothes. As well, the court was told that Ms Trad had a second job when she was hired by Kidstart to make supervisory visits to their centres.
“Kidstart’s failure to adequately supervise sleep practices at the Rhythm and Rhyme centre was a factor in Jack’s death,” Mr Lee said.
